HOUSECALLS

BODOCS are pleased to offer HOUSECALL, our muscle relaxing, excessive sweating and dermal filler service to your home.

HOUSECALL is available on weekdays from 9:00am to 8:00pm and weekends 9:00am to 4.00pm. The service is currently available in the London and Surrey areas. There is a £100 call out fee to each address ie to treat three people there is just the £100 call out fee. If cancelled with more than 24 hours notice there is no charge. If cancelled with less than 24 hours notice the Housecall fee will still apply. Payment accepted by cash or cheque.

Please read the following questionnaire and EMAIL or CALL us detailing;

-any questions you answered YES to (you may not be suitable for a Housecall)
-your name, age and address
-time and date you wish to have the Housecall
-mobile and landline
-number of areas you are interested in receiving muscle relaxing treatments

QUESTIONNAIRE:

• Is there any possibility that you are pregnant?

• Are you breast feeding?

• Have you recently been treated with any other dermal filler on your face?

• Do you have any permanent implant(s) at the site(s) to be treated?

• Have you undergone laser skin resurfacing or received a skin peel in the past six weeks?

• Do you suffer from facial herpes simplex or have any active skin conditions, e.g. acne or psoriasis?

• Do you have or have you ever had skin cancer in the proposed area of treatment?

• Have you previously been treated with dermal filler or muscle relaxing injections and shown hypersensitivity to these?

• Have you received Roaccutane treatment in the past 12 months?

• Do you suffer from any known allergies?

• Do you have a history of anaphylactic shock (severe allergic reactions)?

• Are you taking aspirin, steroids or anticoagulants?

• Are you having treatment with antibiotics?

• Do you suffer from any illnesses, e.g. angina, epilepsy, diabetes, HIV positive, hepatitis, auto immune disease (e.g. rheumatoid arthritis), depression or myasthenia gravis?

• Have you recently undergone major surgery?

• Are you currently undergoing dental surgery?

• Do you suffer from fainting or low blood pressure

• Do you suffer from keloid or hypertrophic scarring?

• Do you have a needle phobia?

• Are you prone to bruising?

• Have you recently been exposed to the sun or sun beds?
